Abstract
Objective
Emergency clinicians face elevated rates of burnout that result in poor outcomes for clinicians, patients, and health systems. The objective of this single-arm pilot study was to evaluate the feasibility of a Transcendental Meditation (TM) intervention for emergency clinicians during the coronavirus disease 2019 (COVID-19) pandemic and to explore the potential effectiveness in improving burnout, sleep, and psychological health.
Methods
Emergency clinicians (physicians, nurses, and physician-assistants) from 2 urban hospitals were recruited to participate in TM instruction (8 individual or group in-person and remote sessions) for 3 months. Session attendance was the primary feasibility outcome (prespecified as attending 6/8 sessions), and burnout was the primary clinical outcome. Participant-reported measures of feasibility and validated measures of burnout, depression, anxiety, sleep disturbance, and stress were collected at baseline and the 1-month and 3-month follow-ups. Descriptive statistics and linear mixed-effects models were used.
Results
Of the 14 physicians (46%), 7 nurses (22%), and 10 physician-assistants (32%) who participated, 61% were female (n = 19/32). TM training and at-home meditation practice was feasible for clinicians as 90.6% (n = 29/32) attended 6/8 training sessions and 80.6% self-reported meditating at least once a day on average. Participants demonstrated significant reductions in burnout (P < .05; effect sizes, Cohen's d = 0.43–0.45) and in symptoms of depression, anxiety, stress, and sleep disturbance (P values < .001; Cohen's d = 0.70–0.87).
Conclusion
TM training was feasible for emergency clinicians during the COVID-19 pandemic and led to significant reductions in burnout and psychological symptoms. TM is a safe and effective meditation tool to improve clinicians’ well-being.

Question: Maharishi, why is it so important to be regular in meditation?
Maharishi: Considering only one aspect of TM, it is the rest that it gives. TM produces rest. Now we know from habit—if we are habituated to sleep at ten o’clock every night, and if we are regular in time with our rest, getting up at five o’clock in the morning—if we are regular with the hours of rest, then health is better. Regularity in habits of eating and resting adds a great deal to good health.
When the mind settles down during the TM technique, the body gains a profound state of rest
Meditation also is a procedure of taking rest—much deeper than deep sleep. So if our system is habituated to take deep rest in time every day, that will be a great help to maintain good health. Regularity is very, very helpful for maintaining good health.
It’s very simple to follow, just considering only one aspect of rest, because it’s such a profound rest. And if we expose our system to this profound rest every day at the same time, then the cycle of rest and activity and freshness gets established. All effort should be made to be regular in meditation. And according to our routine, we make the time ourselves, and that is the time, and that’s it

Wounded Warriors Canada is a national mental health service provider using clinical best practices and evidence-based care to create an environment of compassion, resiliency and hope for Canada's Veterans, First Responders and their families. Funding from the Veteran and Family Well-Being will allow Wounded Warriors to implement the “Warrior Kids Program”, which designed for children exposed to the secondary effects of trauma as a result of a Veteran or First Responder parent struggling with the effects of operational stress injuries such as PTSD.

In 2019, he also trained in Transcendental Meditation, a simple mental technique that teaches practitioners how to settle their minds and their bodies, and reinforce the mind-body connection. “I did lots of research into different ways to care for your mental health, and I read that meditation can be good for you,” he recalls. “After meditation, I feel so chill, so incredibly peaceful.
